Northeast Florida Toy Dog Club Application

Please note: Club dues are $15.00 per
person or$25.00 per family household per year and must accompany this
application. All applicants for
membership must attend at least one general meeting of the Club as a visitor
prior to the submission of an application for the first reading. A vote by the
membership will be taken following the second reading of the application at
which time applicant may not be present.
